FRESHMEN/ TRANSFEREE-
HIGH SCHOOL
Have completed the general curriculum prescribed for a government-recognized school- Elementary Report Card
- Student Permanent Record (SPR) Form 137, Learners Registered Name/Number (LRN)
- Certificate of Good Moral Character from the previous school attended
- Birth Certificate on Security Paper from the National Statistics Office (NSO) (original and 2 clear photocopies)
- Three (3)copies of recently taken passport size white background with collar

High School Graduates who wish to enroll in PCT must:- Have completed the general curriculum prescribed for a government-recognized school

- Form 138-A (High School Report Card)
- Student Permanent Record (SPR) Form 137, Learners Registered Name/Number (LRN) for High School Students
- Certificate of Good Moral Character from the previous school attended

- A photocopy of marriage contract on Security Paper from the NSO (for married students only)
