        1. Form 138-A (High School Report Card)<\/li>\n
        2. Student Permanent Record (SPR) Form 137, Learners Registered Name\/Number (LRN) for High School Students<\/li>\n

                  RETURNEES AND CONTINUING STUDENTS<\/strong><\/h2>\n

                  A returnee is a student previously enrolled in PCT and did not enroll in other schools.
                  \nReturnees and continuing students in PCT must submit the following requirements:<\/p>\n

                  1. Student Clearance<\/li>\n
                  2. Evaluation of Grades<\/li>\n<\/ol>\n<\/div>\n
                    \n

                    CROSS ENROLLEE<\/strong><\/h2>\n

                    1. Endorsement from the current registrar<\/li>\n
                    2. Payment of Miscellaneous and Tuition Fee<\/li>\n<\/ol>\n<\/div>\n
                      \n

                      FOREIGN STUDENTS<\/strong><\/h2>\n
                        \n
                      1. Pre-Requisite for Enrollment
                        \nA prospective foreign student-applicant for the tertiary or college level should be a graduate of high school\/secondary school or its equivalent.<\/li>\n
                      2. General Admission Requirements
                        \nThe student applicant must:<\/p>\n
                          \n
                        1. Pass the interviews of the Program Head and the Dean of College<\/li>\n
                        2. Submit a Certificate of Good Health issued by a PCT accredited physician (with laboratory results)<\/li>\n
                        3. Proof of Documents on Permanent Residence<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/div>\n<\/div>\n

                            \nThe student will inquire Courses, Fees, Admission Requirements, Class Schedules, Crediting Procedure, Blended Learning classes to the E-STAFF<\/li>\n
                          • Step 2 – Cashier<\/strong>
                            \nStudent will pay enrollment fee and get E-KIT<\/li>\n
                          • Step 3 – (Go Back to) E-AREA<\/strong>
                            \nStudent will submit filled out forms and admission requirements to E-STAFF for checking.<\/li>\n
                          • Step 4 – IBU<\/strong>
                            \nStudent will order: Uniforms, pins, nameplates, books Student will process ID Card<\/li>\n
                          • Step 5 – OSS<\/strong>\u00a0<\/span>Student will meet the Guidance Officer for orientation. Student will be given a copy of the following documents:\n
                              \n
                            1. Statement of account Schedule of classes<\/li>\n
                            2. Semi-final copy of crediting form<\/li>\n
                            3. Waiver for admission requirements<\/li>\n
                            4. List of reminders<\/li>\n<\/ol>\n<\/li>\n<\/ul>\n

                              For Transferee Student<\/strong><\/span><\/p>\n
